To save time during your appointment please print out the forms below and complete them prior to your visit. Please arrive at our office 15 minutes prior to the scheduled appointment time with the paper work provided below. The time you have been provided for your appointment is not the time you should arrive at the office but the time you will be brought back to see Dr. Thomas. Also if you have not provided your insurance information to the staff please call and do so. This allows us proper time to verify all of your insurance benefits prior to arriving at the office.
